Zusammenfassung
Übersichtlich werden in komprimierter Form die prinzipiellen Operationstechniken der
Nase (funktionell) und Nasennebenhöhlen (entzündliche Erkrankungen) abgebildet. Herausgestellt
wird die Rolle des Septums für die Funktion, Form sowie die Stabilität der Nase als
zentrale Struktur (insbesondere für die Nasenspitze und den Nasenrücken), sowie für
den Erfolg einer Rhinoplastik. Zusätzlich wird auf den Regulationseffekt der unteren
Nasenmuscheln bei nasaler Obstruktion durch Reduktion des Volumens der unteren Nasenmuscheln,
insbesondere im Bereich der Nasenklappe eingegangen. Eingriffe an den unteren Nasenmuscheln
(zu favorisieren ist die submuköse Muschelchirurgie und Laterofrakturierung). Für
die Polyposis nasi besitzt die HNO-ärztliche Endoskopie bei Diagnostik, Therapie und
Rezidiverkennung den größten Stellenwert. Oft bleibt bei ausgedehnter Polyposis nasi
als Therapie nur noch die endonasale Nasennebenhöhleneröffnung. Die OP-Indikation
wird erst nach Ausschöpfung medikamentöser Therapien gestellt. Die FESS (Functional
Endoscopic Sinus Surgery) dient dazu, die nasale Obstruktion zu beheben und Symptome
zu lindern. Eingegangen wird auf die postoperative Nachsorge und als wichtigste Rezidivprophylaxe
die gezielte postoperative Nasenpflege.
Abstract
A compromised overview of surgical techniques regarding the nose (functional) and
para nasal sinus inflammation surgical treatment is exposed in this article. The nasal
septum is within the focus for function, form and stability for the nasal structure
(especially for tip and back of the nose) and for success of a rhinoplasty. An important
role play the lower nasal turbinates regulating nasal air flow and thus having a great
effect after turbinate surgery (submucosal resection and lateral fracturing).The endonasal
endoscopy is of utmost importance for diagnosis, therapy and detection of recurrence.
In severe cases of nasal polyps, functional endoscopic sinus surgery (FESS) remains
the ultimate therapy. However, the indication to operate will be carried out after
exhaustion of medical treatment. The most important recurrent prophylaxis for rhino
sinusitis and nasal polyps is an appropriate post operative nasal care.
